This statement should be made as a separate file and attached to the IAPMEI form, where it says "Add declaration of tax obligations fulfilled".

Application to transfer money to a Portuguese bank

This statement should be attached to the certificate of available funds in the bank. But only if the money is not in Portugal. If the amount is in rubles or dollars, then you need to indicate the same currency and next add the equivalent in euros at the current exchange rate.
